The city of Bedford retains all birth and death records for persons who were born or died in Bedford Hospital or withing our city limits.
Our records date back to 1908 to current. When making inquiries about vital statistics, please send all email to: water@bedfordoh.gov If you are obtaining a copy of your birth or death certificate, the following information will be needed: BIRTH RECORD Name of Person Date of Birth Mother's Maiden Name Father's Name $25.00 per copy payable by money order and include a self-addressed, stamped envelope Mail to: City of Bedford Vital Statistics 165 Center Road Bedford, Ohio 44146 DEATH RECORD Name of Deceased Date of Death Funeral Home Name and Address $25.00 per copy payable by money order and include a self-addressed, stamped envelope Mail to: City of Bedford Vital Statistics 165 Center Road Bedford, Ohio 44146 Certificates can also be obtained in person Monday through Friday 8:00 AM-5:00 PM. We do not accept personal checks or credit cards. Acceptable forms of payment are cash or money order. Bedford Vital Statistics Department also handles Acknowledgment of Paternity paperwork. This paperwork is necessary when the mother and father are not married and the father is being added to the birth certificate after it has been filed to the Local Registrar at City Hall. When filing this Acknowledgment, both parents need to be present and need to bring picture identification to be able to complete the form. This can be done during regular business hours of Monday through Friday, 8:00 a.m. to 5:00 p.m.
